- Zamboanga City
No applicants will be admitted beyond the above stated filing period.

Please Bring the Following Requirements
A. Application for examination ( for first time applicants) :
- Birth Certificate from National Statistics Office (NSO) printed on security paper or SECPA
- Marriage Contract from NSO printed on SECPA (for female married applicants only)
- Original and authenticated photocopies of Transcript of Records with scanned picture bearing the following remarks:
- “FOR BOARD EXAMINATION PURPOSES ONLY”
- “GRADUATED OR CONFERRED” Date of Graduation (month/day/year)
- S.O. number (if applicable)
- Certificate of Exemption from S.O. – for applicants from accredited schools
- Separate copy of CHED S.O. with signature of approving authority. Extracts not acceptable.
- Diploma (if exact graduation date is not indicated in transcript of records)
- Latest three (3) pieces identical passport size colored pictures with complete name tag, white background and in proper attire (with sleeves)
- 2011 Community Tax Certificate (CEDULA)
- Php 900.00 – examination fee
B. Application for examination ( for repeater applicants) :
- Photocopies of any of the following documents used in the previous examination taken, if available:
- Application Stub,
- Report of Ratings, or
- Official Receipt
- 2011 Community Tax Certificate (CEDULA)
