Professional Regulation Commission announces the schedule of registration for passers of LET March 2012 Licensure Examination also known as March 2012 Teachers Board Exam passers. This announcement are only for the passers from Manila officially  posted by the PRC.

Requirements:

• Duly accomplished oath form and board certificate form
• 2 pcs. colored passport size or 2 x 2 picture, white background with complete name tag
• 1 pc. 1 x 1 colored picture, white background with complete name tag
• Recent Cedula
• Documentary stamp for the oath form
• Initial registration fee of Php 600.00 and annual registration fee of Php 450.00 for 2011-2014

          ** Application stub / Notice of admission (for identification)

          ** Black ink regular ballpen (for filling out the registry)

Lucena March 2012 LET Passers Schedule of Registration and Oath Taking

The Professional Regulation Commission - Lucena Regional Office announces that the schedule of registration for March 2012 LET board passers is on April 26 to June 11, 2012 and onward.

Requirements for registration are the following:

• 3 pcs. passport size colored picture in white background with complete name tag (Last Name, First Name, Middle Name) e.g. GAGALAC, ARNEL ROSARIA
• 1 pc. 1 x 1  colored picture in white background (same shot)
• Community Tax Certificate (Cedula)
• Payment: Registration Fee - Php 600 / Annual Fee - Php 450.00
• Metered documentary Stamp

Step in Registration:

1. Present NOTICE OF ADMISSION / APPLICATION STUB or any VALID ID at window 8 (for verification of rating) to secure OATH FORM.
2. Check your NAME and DATE BIRTH from your verification slip, let us know if there is any discrepancy.
3. Fill-out the oath form properly.
4. Pay annual fee / registration fee in window 17;
5. Secure documentary stamp the Customer Service Center.
6. Pay OATH TAKING TICKET at PRB Representative
7. Submit the properly filled-out OATH FORM at window 11.
8. Wait to be called at window 9 for the registration proper. 
9. After registration, receive your claim slip. ( Check the presence of the following - registration number and date serial number)

For those who want to apply  for CERTIFICATION of PASSING and RATING:

    10.  Secure ACTION SHEET from the guard.
    11.  Pay certification at window 17 (Php 75.00 per copy)

    12.  Secure documentary stamp the Customer Service Center.

   13. Submit action sheet, official receipt, & photocopied verification slip and claim stub at window  7.

NOTE: Please bring your Notice of Admission / Application Stub for verification.

OATH TAKING SCHEDULE:

Oath taking will be held on 8 A.M., June 12, 2012 in at Sacred Heart College Gymnasium. Attire should be Smart casual / Semi-dress / Sunday Dress.
